Photo Identification- Please be prepared to present a valid driver's license, state identification card, military identification card, green card, alien registration card or passport. Out of State photo identification will be accepted.  Applicants must present a valid photo identification to the Live Scan Operator.  Expired or photocopied identification will not be accepted.

Request for Live Scan Service form-  This is provided by your employer or requesting agency.  You must have three (3) copies at your appointment. Please have all forms filled out prior to your appointment. We do have "blank" live scan forms for your convenience, the applicant will need to provide the ORI / and or billing information provided by your requesting agency.

Ink Card(s) for Hard card printing-  These cards (FBI  258) can be provided by you or us.


Type of Payments Accepted-  We accept exact cash, check, VISA/MASTER CARD credit/debit cards, money orders, cashiers checks, and billing accounts.
